Rageblade, Runaan's Hurricane, Frozen Mallet, Rapid Firecannon, Black Cleaver, boots

His Mini form W damage is %health magic damage so it scales really well with rageblade and hurricane puts that damage into an AOE. Makes him squishy as heck but it's always fun for some cheese
