He loses out (more or less) on 3 damage since he has no early game AD ratios to speak of (and the AD ratio on his Ult is pretty meh one way or the other). In exchange for that three damage he gets a ton of regen and an extra potion to help him survive the lane phase.

They should've restricted the bonus damage to minions as they originally intended to, imo.

Jinx has her ZAP! and the rockets augment her AD. Trist has her bomb, of course. Vayne her tumble and condemn. But I would agree on Twitch. The odd AD Kennen, too.
I think I know why they took the route they did. They changed it from +% health regen and minion only damage to +flat regen and OnHit damage. This will make it a much more powerful early game tool for surviving tough lanes, but it doesn't scale as well as Blade's raw AD and Lifesteal. They wanted to make it an option for a wider range of Champions.
It is, basically, Doren's Lube.

Either the patch notes or wrong or the tooltip didn't get changed in game.
Time to find out which it is.
