I don't have a PBE account so I can't test it for sure. Since the description says "attacking the same champion" I'm assuming it works on both basic attacks and abilities?
So how does this keystone interact with AoE attacks? How much stacks do I gain/lose if I damage 3 champions at the same time?

Riot really needs to get their keywords down. They specify "basic attack" in some places, but just say "attack" in others - its very important to get your keywords down because it can cause a ton of confusion.
In path of exile they have ; Less Reduced More Increased as modifiers for things... Less and more are multiplicative while reduced and increased are additive. They keep their keywords VERY straight.
