You have life steal via You don't really need much more than that for a long while except on special snowflake champions.

There are some ADCs who can't afford to buy lifesteal items that early. and for example tend to like to build more crit items and pass on Lifesteal items altogether. These champs also tend to take Warlords Bloodlutst to help cover their lack of Lifesteal itemisation.
