Its based on the champion. If you're a ranged ADC like twitch or caitlyn, it procs your passive for each bolt. But for someone like ezreal its useless because he's reliant on skills more than anything.

Whatever you do though, do not buy runaans on a melee person.

It only affects the primary target you hit. Not the side bolts
