I went 23/5/14, got over a 130+ creep score, and helped push every lane. And then it slapped me with an A+ proving that riot don't actually care about strategic play and just care about arbitrary things you do in the game. Like killing X amount of minions and killing X amount of turrets and dying less than X amount of time.
Just a brief glance at your history, the 23/5/X game you mentioned isn't there, but I do see a pattern. You barely ever ward... making this a complaint of the grading system not giving you a "HEYYYY you perforced at the top 5% players this game" without even participating in something as important as vision. 0 pinks ever bought so far too.
