Play a support and buy a sightstone, then get carried. At the end of the match you'll have an S+.
High Kill participation level, High CS level, Ward Placement, ward destruction, High Damage, low time spent dead, low damage taken.
Generally, each class requires different levels in each of those categories in order to get S+
Supports need high Kill participation % (Around 80% or higher) at least 1 ward per minute, at least 5 Pinks purchased per 15 minutes, and low death count.
I suspect S+ is the one grade we know the most about, since in one of the podcasts (the hextech crafting one I believe), they mentioned you have to perform in the top 2% of all players on that champion in that role. This doesn't actually tell us anything useful, but at least we know where the cutoff is.
The metrics by which they determine performances though they're quite tight lipped about since they don't want people to be trying to game the system (e.g. with spamming useless wards or drawing out the game to bolster their kda or cs/min or whatever), but they assure us that they align very closely with 'being a good league of legends player'.
The key thing to keep in mind though is that your performance is relative to all players of that champion in that role, not just the players in your particular game or in your particular tier, and so an S+ worthy performance on one champ in a given role might only be an A rank if you performed the same way on a different champ or in a different role.
As someone who has 25 S+ ranks on Rek'Sai top, I can't pin it on one thing (though I'm bad about warding, so that's not a heavy factor). It's usually in games that I stomp my lane opponent, take several towers, roam a lot for helping my team, and die little. I probably get a heavy push because I heal a shitton more than a normal rek'sai because of my hydra and BT/DD buys. In general, it's about doing badass destructive things.

No seriously, you need a pretty good score.
The ranks are based on the performance of other people playing that champ.
So if more other players are getting 250 CS and going 10/3/10 that is called a C You need to do better then that to get better then a C
Some champs a ton of people play and play badly so they are easier to get S rank
Champs with 45% win rate are easier to get S class then champs with 54% win rate.
I have gotten a few S+, they aren't that great
