Do you have chests available to receive (check your profile)?
Did you already earn a chest with that champion?
"Hextech loot"? Only chests are earned by S ranks, not keys or anything else.

Exactly what Amelie said. You can check your profile to see if you have a chest available. You have a chance at getting a key fragment per WIN. You can also check your champion list to see if you've already earned a chest on that champion. There will be a fancy little frame around their portrait, and if you hover over it you will see the highest grade you've earned this season. Remember, you're only able to earn 1 chest per champion per season.

Check two things. First, do you have a chest available? (Check in your profile). Second, have you already earned a chest with this champion this season? (Check the champion tab, and champions who have earned you a chest have a different border). You can only earn a chest if you have one available, and if you haven't earned a chest with that champion yet.

Ooooh right. Currently have 4 chests and need keys. How do I earn the keys again?

You have a *chance at earning key fragments per WIN. After you get 3 fragments, you can craft them into keys.

Every win gives you a chance of earning key fragments that can be combined to make a key. Playing games in parties increases your chances of getting key fragments. As you earn more key fragments, the chance of you winning a key fragment in any individual game decreases. This chance resets each month.
