[Champion Concept] Fester, The Plague of Noxus
This was one of the first concepts I came up with, and I'm pretty proud of it. It might be disgusting enough to be off-putting for some people, but I thought the kit was pretty unique and could be quite amusing if the graphics are done well.
Fester is (very loosely) a human. You'd probably compare his appearance to a zombie, but he's not actually dead. Just extremely sick. His abilities infect enemy champions. And now to the fun part: his kit.
Passive: Plague

Fester periodically infects his targets with a contagious plague on his next ability use or basic attack.

Infected champions take extra effects from Fester's abilities, and they lose 1% of their maximum health per minute for each stack of Plague (up to 5).

Each stack of plague increases the infection radius around that champ. Infected champs spread the plague to their healthy allies, giving them however many stacks of plague they currently have. The more stacks of plague an enemy has, the stronger the bonus effects from Fester's subsequent abilities.

To clarify, stacks of Plague do not all spread to everyone immediately. If Fester is laning against Ahri and has 3 stacks on her, and a healthy Lee comes to gank, Ahri spreads her 3 stacks to Lee (if he gets too close), and they each have 3 stacks now. If Fester then hits Lee with another ability, Lee gains a 4th stack of Plague, but will not transfer it to an already sick Ahri. Now if a healthy Varus comes up from bot and gets too close to Ahri, Varus will get 3 stacks of Plague but will not gain a 4th one if he gets too close to Lee after that.

Stacks of Plague do not wear off with time. All current stacks are removed by returning to base, cleansing, eating an orange etc. Alternatively, there could be a new item in the store called 'Antidote' or something and if you use it, it makes you immune to poisons and Plagues for the duration. (This would work for Singed, Teemo, Fester, Twitch, Vlad's ult, etc.)

Fester sneezes violently, hitting a single enemy with a disgusting gob of infected mucous. If the enemy is facing Fester when hit, they are momentarily blinded.

Plague bonus: Weakened enemies are knocked back by the force of the sneeze. (More stacks = knocked farther back.)
W: Fever (click target)

Fester besets a single enemy with a debilitating fever, dealing initial magic damage and then drastically reducing their magic resist for the duration.

Plague bonus: Infected enemies tire easily. Fever reduces their mana or energy regen for the duration. (More stacks = less mana/energy regen)
E: Reek (toggle)

A sickening stench surrounds Fester (at a low mana cost per second), periodically slowing a random nearby enemy as they gasp for fresh air. (Toggling this off and back on again does not reset the cooldown on the slow.)

Plague bonus: Infected enemies are overpowered by the stench and are stunned briefly. (More stacks = longer stun)
R: Blow Chunks (aimed cone)

Fester vomits over a large area in front of himself, dealing initial magic damage to those hit, and then low magic dps for enemies who remain in the puddle of vomit.

Plague bonus: For each stack of Plague near Fester, he vomits a chunk which reduces armor when stepped on. (Chunks are destroyed when stepped on)
Extra Notes:

Fester's basic attacks are melee. He wipes his filthy hands on his enemies to try to infect them.

Hidden egg: Volibear and Cho'Gath (and any other champs with a bite?) take a stack of Plague if they bite Fester. Vladimir takes a stack of Plague if he uses Transfusion on Fester.

Fester is an AP mid (or maybe top?) champ.

Fester's passive is intended as a sort of divide-and-conquer thing for team fights. Enemies will not want to get too close together because of the risk of spreading the Plague.

I'd like to thank someone I played with for the name of this champ. I originally named him Retch, but before posting I played with somebody whose summoner name involved the word Fester, and it seemed much more appropriate.
