Zac Doesn't Retain 5th Dragon Buff when killed in his Passive form

I recently played a ranked game where my team got the 5th Dragon Buff. This buff is supposed to be retained even after death, but when I died in my passive form as Zac , the buff was removed. I think this happens when Shyvanna is in Drag form as well.

Could happen for other form-switching champions.
