Fiora Double Vital [Gameplay]

This bug has been around for a few patches now...I ONLY play Fiora with the Project skin and I have literally never played her with any other so this bug may be exclusive to project. Basically what happens is sometimes, a champion will have two vitals instead of just one BUT one of them is a "phantom" vital meaning that when you hit it you do not get the normal vital bonuses (the other one functions like normal). This gets extremely annoying as if you guess the wrong vital, you just wasted lunge and now your opponent can trade with you for free. Because I am an absolute buffoon, I always forget to take screenshots when this happens(this usually happens 1-2 times in the laning phase per game). The only one I have is of a Renekton but one of the vitals was almost completely gone by the time I had the presence of mind to take a screenshot.

Steps to Reproduce: Honestly not entirely sure but(AND THIS IS A COMPLETELY RANDOM GUESS) it may be caused by people walking out of vision at the same time as your vital times out. I don't really know what causes it but playing a few regular Fiora games should show the issue.
