Unknown death reason (death details)

Ok, so during a battle, an Comes near me, and i die, she didn't attack and i didn't have an ignite or any DOT effect on my, curious, i check the death report and i see that the last thing that hit me with just enough damage doesn't have an icon (see through) and only says the name of the player.

I don't know if this is supposed to be some kind of general glitch, a hack or what (thats why im not posting the name of the player). Is there a source of damage known for displaying an invisible icon with no text ?

Any idea of what might have happened ?
