If you use the same champion and same skin in more then one game in a row, you will not get the skin you chose in second and subsequent games. This has happened to me many times and people I play with in team builder have complained also. If you need any additional information please let me know.
Happened to me last night with Sona and Vi. I didn't notice that choosing the same skin had any effect, it just seemed like every other game refused to load my skin.

yeah not sure if its all champions but Nami has 2 classic skins. The first 2 if that helps. Any game mode here as well
Running into this issue in Team Builder as well. To ensure that I get the skin I want, before every match I select a different champ before going back and switching the champ I want. It then defaults to the classic skin, and I then select the one I wanted. It's some extra steps, but at least you get to use the skin you want to.
