Queue Bug?

I queued up for Ranked and kept waiting until 3 minutes of searching, then canceling and restarting the search. I don't have low priority queue, so that's not the reason why. Then I reload the client, go to search again and it says I have been banned for 5 minutes for not selecting a champion or declining too many games. But I never even got the "Accept" or "Decline" option.. What's up with that?
