Also when LB killed or was killed, the Heal icon showed up in the announcement.
For a second, I was like, "what? I don't see anything wrong!" and then I saw the heal icon and laughed.
I heard this happens if you minimize your game during a match or during load screen. I one time had a top laner use ignite and from that point on their icon was the ignite icon. It was mildly entertaining once I got past the confusion.
