Katarina Ability Bug

Okay, so this has happened to me twice now. For some reason that is completely unknown to me, Katarina can be bugged to where her abilities will not fire, at all.

In game, I E'd onto enemy, W'd, then Ult'd. After I came out, I tried to Q someone else, though the ability didn't fire (it was off of cool down from reset). I could still E and W, but my ULT, Q, and even the RECALL no longer worked.

How to replicate? I have not a clue, though I would guess a quick use of her abilities with a reset afterwards. I really have zero idea on how this happens and this is simply my biggest guess.

It's an extremely game hurting bug mainly due to the fact that it is a permanent bug that will only be fixed via a disconnect and reconnect. It's game hurting due to you not being able to do anything unless you DC, thus taking a penalty to your account. It hurts your game and it hurts your reputation.
