I was playing a Ranked game as Fiora using the Project: Fiora skin, and had just fought Zed. (This includes him using his ult. Just to include every detail.) I killed him, then teleported bot lane to join my team to push for the win. When the teleport finished, I was in the enemy fountain and died instantly.

I heard someone doing this on another champion, and they think it was triggered when they clicked their Hextech Protobelt. You didn't happen to Q when trying to TP or anything like that did you?
