when are instantaneous things going to actually be instant

take things like flash, old soraka's e, fiddlesticks fear.

too many times i've seen that i died to another instantaneous ability even though my ability went on cooldown. it should not be on cooldown because there was no way to cast it in the first place.

if both abilities in question are instantaneous and prevents the opponent from casting an ability, then the abilities are clearly not instantaneous to begin with.
