Sometimes, Karthus E doesn't activate when I push the button.

During Teamfights, I sometimes try to activate E and it simply doesn't activate until I activate it again. Is it because the button doesn't register? is there some sort of spaghetti code that I don't know? Is anyone else having this problem?
