No chest earned for first S- on champion

I just had a game as and in that game I got my first S- on him and at the same time I got my lv5 mastery on him. at the post game screen only the lv5 mastery message showed up and no chest was gained. here is a screenshot of the post game lobby if it helps. http://i.imgur.com/5xWZhrF.jpg

Did you play a premade with someone who got an S on Vel'Koz?

Check your champion page; if his picture has a gold border around it, you've already gotten his chest.
