No chest earned for first S- on champion

I just had a game as Velkoz and in that game I got my first S- on him and at the same time I got my lv5 mastery on him. at the post game screen only the lv5 mastery message showed up and no chest was gained. here is a screenshot of the post game lobby if it helps. http://i.imgur.com/5xWZhrF.jpg

Did you play a premade with someone who got an S on Vel'Koz?

Check your champion page; if his picture has a gold border around it, you've already gotten his chest.

Just checked it is golden, but I had never gotten an S on velkoz before nor did I duo with anyone and got a chest as velkoz.

In my experience if you are in a premade and someone gets an S rank on a champ, you don't get credit for an S rank on that same champ, you get it on the one you were playing instead.

Can confirm this. The champ you were playing gets the chest, not the one that got the S-rank.
