Kha'zix's evolutions don't display

With the new buff/debuff toolbar update, Kha'Zix's evolution icons (not the icons for choosing the evolution you want, but the icons that should show on your buff/debuff toolbar, next to your 'Unseen Predator' icon) currently do not show the icon when they appear after you evolve. You can reproduce this simply by evolving one of his abilities and observe that no icon shows up.

Also, where can I give feedback about the new placement of the buff/debuff toolbar. It's very disruptive to my play :(
