Issues with Playing in General

I've been playing for nearly two years, and I'm going to be finishing this season at the bottom of Bronze 4 (not an intentional down-slide). I started ranked around May/June and got placed in Bronze 1, then the last few months have been me going up and down and up and down, going further down half the time. After a couple of notably bad matches lately, with a leave due to the coffeehouse I was playing at closing and me losing track of the time, I figured I'd post my match history and see if I could get other perspectives on how I've been trending in general (W/L ratio, K/D/A per game, etc). Am I doing OK, am I dying too much, crappy CS, that kind of thing.

Let me know if it only shows ranked games to people other than myself, I've been playing normals through Team Builder lately.

Every time you die, think of what YOU did wrong, rather than what your team did wrong. 99% of the time there's something you could have done better. Just keep this in mind and you'll get better.

I can't tell you if you're playing OK or not. That's up to you. Honestly, it's a video game, and unless your goal is to earn money playing League, the only thing that really matters is whether or not you're enjoying yourself whilst playing. If you are, then you're playing OK. If you're not, then you aren't playing OK.

Now, as far as increasing your win rate goes, I think I can help you there if that's your goal.

Your performance seems to be significantly better on ADC than on any other role. So try to always get ADC.
Your Pantheon jungle seems fine as well. I'd consider using him as a 1 trick pony kind of champion when you jungle.
Stop building coin on supports. The item is a noobtrap. You're pretty low ELO, so (unless you ask for it) I won't bother you with long explinations of which starting setup to get in which situation. Just go for the following cookie cutter openings: Spellthief on Morgana, Targon on Thresh and Leona. Spellthief on any other mage-like support, coin in Janna, Targon on everybody else.
Don't always rush Randuin's as 2nd item. CDR tends to be better, especially on Thresh. If they have a lot of physical dmg, get an early Glacial Shroud.
On your ADC's: more shivs, less phantom dancers. I'll spare you the details unless you ask, again.
NEVER buy cloak of agility first item. Literally never.
