How do I get good at this game

Well, first of all, you made it to silver in a decent amount of time, so you don't suck. Most of the people at Plat and above have spent a very large amount of time playing this game, because it causes improvement. I would be gold on my main last season, had I not received (a well deserved) ban. Instead, I am gold on my smurf while my main sits in Silver 2. This is moving up from Season 3 Bronze.
Things that helped me move up.
Main a champion and a role. You would benefit from focusing on 2-4 champions and identifying your best champion in each role. Explore what it is you do best and then try to do it as often as possible.
When you're forced out of your best roles, understand and accept that you will not be carrying the game-- instead, focus on not feeding. Your Jax, Yi, and Garen stats show that there are games where you are feeding out of control. When you're put in these situations, do everything you can to be a neutral or only slightly negative influence on the game's outcome. One thing these champions have in common is that they do not have CC. If you're forced out of your comfort zone, pick a champion with great CC so that no matter what happens, you will be useful in teamfights.
Have objectives for what you are practicing during normal games, for example, if you're practicing jungle, have either target #s for your CS count or for your gank counts. Practice different approaches. If you're practicing support, try to focus on your ward uptime, on play-making, or on lockdown/peeling. Go into games with things you want to do and then try doing them.
Watch replays of your games using lolreplay or by recording them. It can be painful to watch yourself, but when you see times that you made bad calls, it will really help you make better calls later or identify the things you need to fix.
Smurf. The reason to smurf is not to beat up low level players and feel good about yourself, that's pathetic. The actual reason to smurf is because it teaches you how to carry. It doesn't matter if you go 25-3 if your team still loses-- you need to learn how to translate that into a win by taking objectives and advancing lanes. In one game that ended 49-56 with us (49) winning I was 45 of our 49 kills. We still almost lost, but because I took out everything that stood in my teams way and let them push, we were able to win. Learning to carry is vital and carrying is really only possible when your own play vastly outmatches others.
Learn how to make calls. If you kill 3 of their team members and you're all up and in good health, it's probably time to grab baron or dragon and gain more vision on their side of the map. If you're down 2 team members and don't see any of theirs on the map, it's time to go check Baron/Dragon and see if you can make a play on it.
Leadership. If you're doing really well in the game and you ping for assistance, people will usually listen. If you're doing poorly, apologize and defuse anger. Always be willing to take blame, always be a good sport, and always congratulate others when they make a good play. Just typing wp or gj can mean a lot.
Help stragglers but don't invest in lost causes. Do your best to keep people from going on severe tilt.
