You did awesome, but I think you should consider Locket of the Iron Solari instead of Banshee's Veil. BV is a greedy pick for a support, it could possibly block a single skill shot from hitting a teammate, but beyond that, Locket would've protected your team a lot better. I guess you got Trinity Force for the utility and not the damage, but again was a greedy pick. I would've gone Randuin's Omen, or get Frozen Heart if the enemy has a good amount of Auto-attack reliant damage. You can stack BV after Locket if the enemy has a strong AP team and you don't need Armor.
You warded a lot, great job. But you never swapped your trinket for a Sweeping/Oracle's Lens. This should be immediately bought when you get your Sightstone, and try to upgrade at level 9 if gold and itemization requirements allow.
Looking at your build path now, it looks like you bought Sightstone really late and immediately went for Ruby (I honestly rarely need to upgrade to a Ruby till the 4th or 5th item). To get that many wards out, you must have placed a ton down over the cap replacing old ones before the time went up. Focus on making better placements of your wards so you don't have to put more down. And more importantly, you need to get Sightstone on your first back, not at the 23 minute mark. You never got Pink wards either; you can get one early on and depending on where you put it, it might last the whole match and prevent jungler ganks or roams.
I don't know what exactly a good amount of CS should be if you're using Face of the Mountain efficiently, but make sure you're getting as many Siege Minions with it as possible (they give by far the most gold), followed by melee minions.
