How to Make a Comeback and/or Carry

Hello guys, I have heard the phrase "Focus the backline, not the frontline" a lot. However, even when our team kills part of all of the frontline, our team still loses the teamfight... Generally, what could be the case? And additionally, if a bruiser or tank gets fed, as a mid laner like an assassin what should you be doing for your team? Thanks!
