What to do when...?
Honest question here? If you get a team mate who continuously dies and doesen't group. What can the other 4 players do? Looking for an honest answer here.

Try to get objectives and avoid 4v5s.
You could try to group with him so he's not alone, even if he's not in the best place. Or hide nearby and ambush whoever comes to kill him.
Alternatively, if you see he just got ganked by 1+ members of the enemy team, you should do something to punish that... push a turret, force a fight 4v4(3/2/1), get dragon/baron, ward the enemy jungle and catch someone, etc.
