Going in as Thresh in Lane
I poke the enemy ADC when he goes for CS with my Flay passive up and get him to use pots. Ok, so my question is that when do I go in as Thresh? When do I hook?

Like many supports, 
excels in applying pressure by causing problems to marksmen who wish to cs. My best advice for you is to sit in the bush and try to hook and auto people as often as possible without being in an uncomfortable position. A video of C9 Lemonnation giving a gameplay example of this type of playstyle (in this clip he is hesitant to hook because it would allow 
and 
to immediately turn on 
for a kill). Situational events like that clip can come up where grabbing all the time may not be your best option, so try to experiment with matchups and figure out what works for you.
Your main goal is just to give the enemy doubt about going for cs in fear of being hooked or taking too much poke. Hooking in place of an auto allows for your marksman to get hits in as well, which is the optimal situation.

When I play thresh, I usually go in lvl 2 no matter what
