I have been testing every adc champions with Runaan's Hurricane. For Kalista the only situation to go for that is when you choose to learn E first, because when a minion is low and there is a marked spear, E can do damage and be reseted. While in fight Kalista can A enemy champions and E once if one of the minion is going to die. Which provides an literally unlimited E.(If controled correctly

Yeah I tried it too as my final build item when I was just messing around the other day. It seemed pretty nice.
The main thing is that ruunan's doesn't offer any AD or AS or Crit or even Vampiric Ability. Often times there are better items that could fit into that slot on an adc. But it can be an effective item under the right circumstances.
I believe that Kalista's Q will pass through a unit if it kills it, and passes off any spears stuck in it. So if you're fighting someone in a minion wave, and stacking spears on the minions, and line up a Q, you could potentially spread spears from minions to an enemy champ for a pretty massive Q/E. Hard to do, but seems like a fun gimmick.
