I have a theory. What if the elder tried helping Riven shatter her blade and in the act of doing so, the blade exploded (similar in the when in the meeting hall) and the resulting force killed the elder and students. The force may had been so great, it rendered her unconscious, Add both the PTSD and the physical trauma, she may not remember anything anymore.

In the story when the blade was exploding in her cell a part of it went for the elderly farmers throat right?
At the same time riven gets a flashback of the same thing happening but that time the shard actually hit the village elder, Yasuo´s teacher, and killed him right away as far as we know.
And probably all the others around as well.
Then Riven who already has PTSD at the time breaks completely and goes wandering for a bit until she finally collapses on the farmers fields.
This is at least how i mapped things out after reading the story......although i did not account for actual physical trauma when the blade exploded the first time around which is a probable thing in hindsight.
