If you read his story and hear his quotes, it's evident he isn't associated with Noxus or their military and is just a psychotic gremlin that makes his decorative titles up? Riot kept this real ambiguous and it's pretty frustrating to think of just what the hell he is.
He has his own ideas about what Noxus is, and those soldiers weren't it. There's nothing official about his standing or what he does but he embodies Noxian ideology and has done since basically the nation's founding. He fights for Noxus because he wants to, not because of any military or national obligation.
Make no mistake, Kled's been a part of every campaign waged by Noxus.

Whenever Kled kills a trespasser (which is often), he assumes that whatever was once the trespasser's is now his own. That includes military ranks. So his long string of titles is really a list of all the officers he's killed. Kled considers these ranks official but no one else will. He is definitely associated with Noxus though, whether the Noxian upper-class like it or not. Noxus is his home, and he defends his home, from other Noxians if need be.
His titles aren't made up. He takes them from enemies he kills in battle.
It's a Noxian ideology. Because he killed them, it means he's more worthy of those titles than they are, and he takes them as trophies.
