If you want to make new fiddlesticks TRULY horrifying
Have it play back a slightly distorted / twitchy version of other champions' voice lines after it kills them.

after fiddlestick kills a champion one of their quotes gets play next time fiddlestick is seen by an enemy champion in a distortion filter
basically if fiddlestick kills annie then he show up to gank toplane he say "have you seen my bear tibbers?" in a distorded annie voice follow by a laughter
If you want reworked Fiddle to be truly horrifying, prevent players from banning him in Ranked once he gets released and witness true horror of the Champ Select.
there was an inner voice idea for fiddle to play multiple voices for the player but if i remember right that was cut because it was too creepy
the idea sounded similar to senuas sacrifice and i would have loved the fuck out of it but i suppose they don't want 1% play rate of the champion xD

Also, please do something else for the Auto Attack. The Tongue-arm is not horrifying, it's just... weird.
I don't think a scarecrow can be scary even if it has an inner demon, if you never see the inner demon its not very scary at all. People have this misconception and tie horror into meaning creepy+unsettling Sure it can be, but horror should be scary on top of that and even though he is creepy he'll never be scary in the game League of Legends, because there are kids playing this game. No i'm not saying league should change to a more mature age range i'm just saying that I feel that they won't be able to make him truly scary while under the restriction of T for Teen.
That being said maybe Riot Forge could make a horror game around the new fiddlesticks where you play a new protagonist (or a known champion) and are trying to escape fiddlesticks in which they could portray him in a more realisticish less cartoony way without breaking the cohesive lining of why does that one champion look hd when every other character looks like something ripped out of a more edgy pixar production, if that makes sense
don't forget, if he stays 1.25s not moving he must stop moving his body completely and stay still.

I want Fiddlesticks to "leak" future things to come into the game just like Swain did.
Like, if says something about their deepest darkest fears? Like he could mention something about, for example, Miss Fortune losing her crew? This could hint towards a lore champion being released?

Ugh..alright, it is a very good idea but, especially at higher elos, most players (that play their mains) have skins so it would not be niche if I, as God King Garen, died to Fiddlesticks and I heard normal Garen talking.
Maybe add in some static too, kinda, like a fucked up radio voice in some whorer movies.
Fiddlesticks new ultimate (not really a good idea but I try ) : after 2 seconds fiddlesticks teleports behind a chosen champion and fears them while also dealing damage over time but if there were more than one champions next to them they also get feared and take damage over time.
great idea, but they also should take some different animals and nor ravens cause:
Swain: black/red vfx, ravens, demonic
Sticks the Fiddler: black/red vfx, ravens, demonic (?)
