In one of my games, I was told multiple times by 3 other players to hang myself, they called me retarded, made quotes about sexual activities with my mother, and other toxic things throughout the entire game. I reported them all, but I never saw a message that said they were punished. Is it sometimes you don't get the message? I really hope they were punished.

They might not get punished right away so you don't really see it unless it was yor report that prompted the review. Sometimes they need more than one game to get flagged. They will be punished eventually as long as they continue the behaviour, if they stop then it's a win win, they can keep on playing and their teammates don't have to worry about toxicity.
