There is no reform card to speak of, no response from support ticket, no anything not even am I allowed to login to observe any message. Someone suggested to post here to ask in (usual vain) hopes of getting a public response. The only lead I have on is related to a purchase of RP used to change my name, but even that is flimsy.
I don't care if I'm finished for previous misconduct, I just strongly desire clarity as to why it led to this.

well the rp thing is pretty relevant because if you have errors with payment they suspend your account until its fixed.

i did this and got a reply from riot, so they will probably reply to you in a day or so. but dont expect to get a reply or a ticket reviewed for a while, i still havent gotten one yet

I want to know why I also got perma banned. I don't have current access to the email of this account. I never been chat restricted or suspended in game.
I'm hoping it's a mistake because I've never told anyone to go die in chat, and never been racist. Yes I've been toxic but never got restricted for it, so a warning would have been really nice for me to correct myself as a player and all.
Hopefully a riot member replies back to the ticket. I had one reply today that they getting swamped and trying to forward all the tickets to moderation team or whatever.
Anyhow, hope I get my account back, this one here. I would take a restriction if anything because I've never had one before in game.
I didn't get hacked, replay.gg shows my last game played which was me playing of course. Never told anyone my account info and stuff either. So hopefully we get some answers, gotta be patient.
Send in a support ticket. It is possible there was a billing issue; Riot will sometimes temporarily suspend an account while those are sorted out. What message do you get trying to log in?
this is due to a ban wave that targets bots, scripters and other who use 3rd party violations. And since its a botted wave it sends the same message not really specifying why.
What EXACTLY does it say when you try to log in?
If you goto https://repay.leagueoflegends.com/ it will tell you if you owe a balance. So that should let you know if it was RP related.
You are most likely going to have to wait for a response to your ticket which may take a few days.
