Can you do an auto check for IGN when people make new accounts?
It's been 6 years and I'm still seeing people running around with straight out "kys" and racial slurs in their names.

i know man.. when people have kys in their names my skin crawls and my triggerphobia levels increase 3x.

I have no idea what you are talking about. The name check system is fine. Carry on everyone else.
I think a name is only really offensive if it is directed at a certain person or a group of people. If their name has kys, why would that offend anyone? Now if its directed at a specific person or group, I could see how that is offensive. On Xbox live i had to change my name from Stephenizgod (fun fact I changed it back and no one seemed to care) because apparently too many people reported me because I was stomping their ass in some game or something. My name isn't offensive, it can be seen as offensive if you get "triggered" easily, but its not because I am not directing it at anyone but myself.
So unless a name is specifically directed at you in someway, why would their name offend you?
