Was banned once a year ago for being toxic, I reformed since, and today I'm perm banned with no message of why I was banned. I was on a 14 game win streak so why would I be toxic? I haven't been on League in a few days and i come back to find that I'm perm banned...
It does look suspicious that you went from playing a wide variety roles with about a 50% win rate to suddenly just playing top with a champion you've played 7 times before in Ranked over the past three seasons but now have about an 87% win rate on over the last 15 or so games.
This is a situation where some context or background information from you might be useful.

I was given ZERO email and it does not say I was banned by the Tribunal, it just says to check my e-mail.
what were you banned for?
there should be a UTC code when you log in / email. Post that please.
Check for an email that should contain additional details. If you don't have one, submit a support ticket and they'll give you the information.

I was apparently banned for toxicity, or so that is the speculation at this point. I wouldn't say I was toxic enough to go straight to perma-ban.
