Behavior system allowing toxic player to slip through the cracks

So there is this player who was very toxic, so I reported the player and blocked him. A few days later he shows up in my game. I didn't remember it was him but his name was familiar. After he trolls our game, I see in the after game lobby he was already blocked! Here's the kicker, I que up again and this time I play against him! How is this guy still allowed to play games? The enemy team kept asking us to report him for fountain afk and raging. Sounds just like him. Three games and this guy is still allowed to troll the Rift.

My guess as to why he isn't being disciplined is that he is queuing up with two other players. Either that or they got unlucky and they got placed on the same team with him as I did.
