He is 2/14 right now, and has been running into the enemy team for the past 12 minutes. He refuses to surrender and say "we can win".
Is reporting him really gonna be enough to get him banned? Or is it better if I send in a ticket to Riot Games?
People like this should not be allowed to play in this game. He should be removed from it.

That sucks dude, your best bet is to submit a support ticket because that will ensure that they review the game manually. As per usual they can't tell you what the result of the investigation is, but if the person genuinely inted then you can rest assured that Riot will what they feel is just.
i sometimes troll as well , and can't deny is much fun. i would take anivia and wall teammates to death or simply run down under turret and pretending i'm bad. no ban for now eheh
I would use the post-game report, and be specific about why you think he inted. Was he using his SS out of combat? Was he running it down mid? Was he waiting to engage 1v5? Did he buy items that did not go with his kit? Include anything you'd like, but that's just a few I'd add.
