So what is actually considered to be toxic behaviour?

Toxicity is when you bring insulting, overly negative, or unsportsman like behavior into the game.
This includes insults, blaming, non-respectful or non-constructive criticism, engaging in or extending inter-player conflicts, etc.

Toxicity means you're saying something that is degrading the team-working environment. Insulting someone is definitely toxicity. Blaming someone for something isn't toxicity, BUT, if you continue to harass that person over the issue, or argue, or insult, than yes, it's toxic.
I don't blame people for my mistakes, I blame myself and let other people follow suit. If you call another player out on their mistake, they're likely to get defensive.
If you call your own mistake, others are likely to do the same. That's the trick too, when others make a mistake, you want them to reflect and see the mistake so they can fix it in the future. You never wanna point fingers in league.
Imagine:
The wrong way: "This LB keeps roaming and killing me because you can't use pings, dude" -blaming "Nah man, she backed, I'm not supposed to ping when she backs, stop feeding man." -gets defensive over stupid, easy to fix mistake, and continues not to ping.
The Right way: "Shoot, this LB keeps catching me when I push my lane, brand can you ping w/e she leaves lane so I can try to stop feeding her?" "yeah man, I don't want a fed LB, I'll try to remember"
It rarely works out exactly like that but, you'll have better luck trying to 'help' others than argue with them. Never play the blame game, there is always a loser and it'll be on your team...
