While I don't have the answer to the first question, I might be able to answer the second part. Recently I was banned (yes I deserved it); however, for probably the last year I have been consistently toxic in chat. The fact is the system we have is in a poor state. I addresses the issues in an incredibly poor way. If you didn't know, which I hope you do not, the banning experience is not a "waking up" moment. You say "oh that sucks", then you create a smurf and go back to the toxic ways. There is NOTHING within the status quo to address this issue. Even if someone doesn't create a smurf, you never get rehabilitated. You are punished. You are hurt. Then it's over. That's the system. Very few people care about the incentives of accolades as a friendly person. It simply doesn't matter to the vast majority of the toxic community if they get banned or reported, because there are no stakes to it.
