What does it take for a players account to be reformed?
My friend refuses to post it himself and won't stop bitching about how he can't find it out himself so....
What constitutes a players account is reformed and can receive the box's from games

Riot won't say. Your account needs to be in good standing, that's all we know. Just tell him to stop raging in chat and eventually he'll be able to earn boxes.

They don't want you to reform. They want you to get banned and get another account that way you can pay for the things you already bought. Why else do you think there's no story from ANYONE that they were finally considered reformed?
