I used to permaban malphite because hes such bull shit. But then i realized udyr and xin exist so i started banning them. But then i also realized blue ez exists so started banning that. BUT THENNNNN I realized zed with duskblade exists so I started banning that.

Ban who is a problem for your team comp or champions you personally don't like playing against. Play better than whatever else is picked against you
