How does the Grading System at the end of the game work?!

I don't mean to exploit any names, my team was great and the players were awesome. My point is I almost had a perfect game, yet again, on Karma and I still got only an A.. What do I have to do to get an S or an S+? I mean I'm already mastery 7 but being rewarded with an S+ after having a great game makes me feel a little better :D
