So, I was being punched by Garen and Azir, and then this happened. I know that Death Recap needs some work, but this is the first time I have seen this happen. :P

I don't see the problem.
The death recap has been buggy for quite some time. The only real way to tell how you died is to pay attention or record your games.
that happened to me yesterday. i was fighting an ahri, she missed her charm and Q and then she pops zhonyas. about 1 second into her zhonyas i just immediately die, to absolutely nothing.
my teammate was watching and noticed as well. my death screen looked a lot like yours. ahri\s name was off to the side with her foxfire and auto attack damage, but it didnt add up anywhere near my total health. the rest of the screen was blank.
