So I hit a point where I am good enough to not suck as long as I play a champ I know well, but I am not good enough to carry an entire team. I would love an opportunity for someone to help me out, but not sure where to go.

A lot of streamers do coaching, and people post thread on the boards sometimes offering coaching, there are several coaching websites in addition to this.

Cruise the boards and ask around in game. It don't take much to find a high level player to teach and tutor you. Both of my students this season began in low silver where I met them playing on some random smurf. They both showed some talent and were receptive to being taught and so I spent several months working with both in games and sharing my knowledge. Both managed to progress by leaps and bounds, Zeretov managed to claim a diamond ranking by this years end and Mr. Trianglez was 1 win shy of earning his promotion to diamond a month or 2 back and fell just short of claiming those rewards this season.
Just be sure to find some one who's patient and insightful and plays on a regular basis usually within the same timeframe as you. Always pick the brains of high level players to enhance your strategic understanding of the game which is generally the one aspect most lower level players lack, so its important to find someone who enjoys discussing situations and scenarios with you as well.
I won't be taking on any new students until the beginning of next season, like I do every season, but if I remember this in a few months I'll try to give ya a holler or at least pair you with some one I think could help you take your game to the next level.
