I would imagine maybe the whole team chasing a bard and he portals into fountain from outside of the base and throws a Q in the team. Maybe a bit too situation, but we can dream :)

When you jump into a teamfight, and everything happens so fast that you end up killing five enemies on the enemy team that you don't know what the heck happened.

That is basically what happened with my penta.
