I was trying to poke and flash away at the same time. I didn't move my cursor fast enough and wound up flashing into the enemy team.
Flashing an inevitable death. (ex: flashing after being ignited at 10 hp)
mine is when a 
crow or 
ricochet bounces only cause I flashed to close to an ally and the bouncing kills me or said ally :( sorry 
I remember when I was 
and I was killing a Bard, the Bard had 10 health left so I used double up to kill him. He flashed away, used magical journey through a wall and 5 seconds later, I heard and killing spree!
It was very early in the game as I was a level 5

When people flash into a Lux ult. Not only do they die, but they die in a GIGANTIC LASER.

Not my own experience but It's still funny.
I played Annie Mid for some random reason, and even though I was like 6-8 everyone but the entire enemy team flashed away from me even when I was outside of my range.
The enemy Olaf came to mid lane to try to gank me, he was at full health but I thought I could burst him down since he wasn't that tanky. I ended up following him to the River after he failed to gank and as soon as I came out of the brush he ult'ed, flashed, and ghost'ed away from me even though I was just going to ward.

the fact with ahri i have troubles with the very small cooldowns between her ult.
I ult in to dragon pit, infront of 5 enemies.
I throw out my Q and W and E as fast as possible and steal dragon. amazing.

When you use flash to tower dive during laning phase and die without even forcing their flash.
If I fail flash and die because of it, I accept it and move on and try to not do it again.
It's when I flash towards somebody and spastically click to get that last auto attack off only to accidentally cancel my auto attack in the process. My only thoughts when that happens is "If there is a god, kill me now."
Flashing after an AA

Flashing to get a kill you would have gotten anyway. It feels good in the moment but you're probably going to really regret it 2 minutes later when another fight starts.
