Wow, the new Shyvana rework seems great.. It seems to be a 'bit' Op tho
Saw this a couple times too, it's definately a bug. Zac will actually be perpetually bugged for death recap, always having around 2k-3k true damage thrown in there.
I once made a post about it that got to top three on boards hot, but it's not getting fixed.

This bug is literally older then Ekko and it still happens. I still don't know why this has never been recognized or fixed by Riot.
