How the hell does a Nasus mess up so badly that he has 700 stacks instead of 702?

Typically, by that point, ADCs have tank shredding stuffs and can kite the big bad doggo. Nasus is much more effective, late game yes, but earlier late game than a lot of people want to play him. Nasus vs enemy team at full build does have some troubles, regardless of his stacks.
