When you don't pay attention to your character while watching someone else on a different lane, you end up walking into the turret with a low level person taking the kill for it.
Worse, when you try typing something and someone tries to kill you, you get a sloppy reaction of pressing buttons while the chat box is still on, entering the most ridiculous chat and dying for it.
always hooking the first enemy I see. I quickly learned after hooking

shroom and got an Ace. i Cried
Reoccurring fail that happens to me (Thankfully, not too often) is when I get ignited in a fight at like 5% health and try to flash away forgetting that I'm ignited and die anyway.
Usually people are good about timing it and ignite me a millisecond before I flash and by that point it's too late for me.

I was playing Blind pick and picked Blitzcrank, their team did as well. About 25-30 minutes in their team was quite a ways ahead (I think their Blitz was around 4-2-16) with a nearly-completed Mejai's, and plenty of mana/tanky items. I decided to pull him since he was so much closer to us than his team. When I pulled him onto us, he ulted to take about a half of everyone's HP, then their orianna flashed closer and ulted, finishing everyone off at once.
I really couldn't tell you if they killed us faster than it took for all 5 of us to type /ff. Both happened in less than 4-5 seconds.
As 
I ulted and accidently went around the top 
into the enemy tower... everyone kept making fun of me for it all game :(
