I was in an ARAM as Vi, and as soon as I bought a Phage my Q stopped working.
i tried rebinding the key and clicking the icon, but to no avail.
worst part of it was, i was fighting a team with 3 channels ;--;
a fiddlesticks, Xerath, and Vel'Koz.
i sure hope this is being troubleshooted, or atleast known. because it was quite annoying and cost me the game :(
It's happened to me two patches ago. I informed the PBE community before the patch, and I was told that it was being worked on. It seems its a pesky bug. Reconnecting fixes the issue BTW.

Doesn't look like anyone really has an update for us yet on what is causing it or how to fix it :(
